Anonymity will be maintained between the complainant and the alleged offender, except where necessary in a court of law. However, should this matter proceed to Court, you may be required to give evidence as a witness, your name and your filed complaint may become public information.

The personal information being collected on this form, by or on behalf of Central Huron Clerks' Office, will be used only for the purpose of compliance with the Animal Control Bylaw. It will be protected in accordance with the Municipal Freedom of Information and Protection of Privacy Act.
